APPLICATIONS TO SIT FOR THE APTITUDE TEST TO FOLLOW THE DEGREE COURSES OF STUDY IN ARCHITECTURE DEGREE (B.ARCH), DESIGN DEGREE (B.DES), LANDSCAPE ARCHITECTURE DEGREE (B.LA) (FACULTY OF ARCHITECTURE), FASHION DESIGN & PRODUCT DEVELOPMENT DEGREE (FACULTY OF ENGINEERING) INFORMATION TECHNOLOGY DEGREE AND INFORMATION TECHNOLOGY & MANAGEMENT DEGREE (FACULTY OF INFORMATION TECHNOLOGY)

SUBMISSION OF APPLICATIONS
Applicants are required to prepare their own application forms in accordance with the format given below on A - 4 size paper.ÌýSeparate applications should be submitted in respect of each Aptitude Test.
Each application should accompany a paying inÌý voucherÌý obtainedÌý fromÌý anyÌý BranchÌý ofÌý theÌý BankÌý of Ceylon forÌýRs. 300/= drawn in favor of the Vice - Chancellor, University ofÌýMoratuwa,ÌýAccount No. 306836 (KatubeddaÌýBranch)Ìýbeing the fee for the particular TestÌý(ChequesÌý& Postal/MoneyÌý Orders will not be accepted)Ìýalong withÌýcertified copies ofÌýG.C.E (O/L) and (A/L) certificates as applicable and a certificate indicating the result of the common general paper,Ìýby the Head of a SchoolÌý or a Justice of Peace.ÌýThe official stamp of the certifying officer should be placed on each certified copy. If a candidate submit a certificate of G.C.E (A/L) which obtained from the Web Site, the name of the applicant should be mentioned and certified by the Head of a School.ÌýOriginal Certificates should not be sent.ÌýCandidates are required to sendÌýa self addressed stamped envelope (Rs. 40/= worth)Ìý10 ½ x 22 ½ c.m. in sizeÌý(9x4Ìýinch)Ìýalong with the application form. TheÌýcorrect postal addressÌýshould be written as this envelope will be used to post the admission card for the Aptitude Test, to the applicant.

If any difference exists in the name indicated in the application and that in the certificates, candidates should submit an affidavit to confirm that they relate to one and the same person.
The admission letters will be sent to individual applicants who have satisfied the admission requirement for the aptitude test.
The applications of those who do not possess the minimum admission requirements, other requirements to become eligible to sit for the Aptitude Test as per above, those which do not accompany the paying - in voucher and late applications will be rejected.
Applications should be sent under registered cover to reach the Senior Assistant Registrar / Examinations, University ofÌýMoratuwa,ÌýMoratuwa,Ìýon or before 24.10.2012Ìýwith theÌýname of the Aptitude Test applied for, written on the top left hand corner of the envelope.
